Deadline for State Mandated Vaccinations and Wellness Checks is Nov. 30

Winston-Salem/Forsyth County Schools issued the following announcement on November 18.

Nov. 18, 2021 - On Saturday, November 20, 2021 from 9:00 A.M. until 3:00 P.M. the Forsyth County Department of Public Health is hosting a "Back to School Clinic" which will provide families with access to state mandated immunizations and school physicals as well as COVID-19 vaccines for students.  

Appointments are required and there are a couple of ways to sign up:

For Back to School vaccinations use this link to sign up or call 336-703-4273 to schedule School Physicals.

For COVID-19 vaccinations follow this link or call 336-582-0800.  

State law mandates certain immunizations and health assessments for Pre-K,  kindergarten, 7th, and 12th grade students, or any student that is new to public school in North Carolina. Parents have through November 30 to provide updated records of those immunizations to your school. If student records are not updated, per state law, your child will be excluded from school until the records are provided. For a complete list of immunizations, please visit the “Schools” section of the NCDHHS website. You can also speak to your school nurse.
