The facility offered 138 beds, exceeding the Forsyth County average by 32 beds during that period. Summerstone cared for an average of 126.5 residents each day in the first quarter of 2026.
CMS gave Summerstone Health and Rehabilitation Center an overall rating of 3 out of 5—slightly above North Carolina’s nursing home average of 2.9.
Willow Valley Center for Nursing and Rehabilitatio was the largest facility in Forsyth County in the quarter, with 230 beds available, while Oak Forest Health and Rehabilitation took second with 170 beds.
As of Q1 2026, Summerstone Health and Rehabilitation Center incurred one fine totaling $14,511 and one penalty.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
