Oak Forest Health and Rehabilitation ranked as the second largest nursing home in Forsyth County for the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows.
The center offered 170 beds—64 more than the Forsyth County average for that period—and cared for an average of 156.3 residents during the quarter.
With a CMS overall rating of 1 out of 5, Oak Forest Health and Rehabilitation fell below the 2.9 average recorded for North Carolina nursing homes.
Willow Valley Center for Nursing and Rehabilitatio had the most beds in Forsyth County with 230, while Summerstone Health and Rehabilitation Center was third, offering 138 beds in the same period.
As of the end of the quarter, Oak Forest Health and Rehabilitation reported $43,002 in fines across four incidents, resulting in four total penalties.
Forsyth County had eight for-profit nursing homes total during this time.
Estimates from the U.S. Census Bureau indicate the older adult demographic in the United States is increasing, while younger age groups decrease. From 2020 to 2024, Americans age 65 and up grew by 13%, while the under-18 population declined by 1.7%. The nation’s older adult population has seen marked growth over the last 100 years, from 4.9 million in 1920 to over 61 million in 2024.
A 2021 report noted that nearly half of adults in this category—about 28 million people—may need long-term services and support, with more than a third expected to require nursing home care at some point.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
