Willow Valley Center for Nursing and Rehabilitatio was noted as Forsyth County’s largest nursing home during the first quarter of 2026, according to the Centers for Medicare and Medicaid Services (CMS).

With 230 available beds—124 more than the county average—Willow Valley Center for Nursing and Rehabilitatio served an average of 215.1 residents during the period.

The facility received an overall rating of 1 out of 5 from the CMS, which was lower than North Carolina’s nursing home average score of 2.9.

For the first quarter, Oak Forest Health and Rehabilitation was the second largest facility in Forsyth County, offering 170 beds, while Summerstone Health and Rehabilitation Center came third, providing 138 beds.

During the first quarter of 2026, Willow Valley Center for Nursing and Rehabilitatio incurred total fines of $275,518 stemming from five violations and received five penalties.

There were eight for-profit nursing homes operating in Forsyth County, North Carolina, in total.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
